Enhanced Care Management (ECM) is having a Care Team that builds a comprehensive health plan for those with highly complex needs. ECM is a benefit that provides extra services to help you get the care you need to stay healthy. It coordinates the care you get from different doctors and others involved in your care. ECM offers supportive services to address your whole health - to care for your body and mind.

The team can help fill your support needs by linking you with support that can benefit you, such as, but not limited to, housing resources, mental health services, community support, food resources, and physical health needs, varying based on your need.
